Checklists Module

Checklists provides preconfigured, electronic, operator daily checklist inspections as required by OSHA regulation 1910.178. The checklist can be customized to be completed at the beginning of a shift, each time an operator logs on, or by different operators. An inspection is performed by answering the questions on the checklist. If the checklist is not completed successfully, then the truck is locked out.

•Failed = Considered to be critical to the truck’s operation. The truck will not operate if a critical question failed, therefore causing the inspection to fail. The supervisor needs to swipe their badge to unlock the truck.

•Timed Out = The operator took too long to answer the checklist questions. The operator is logged off and must log back on to restart the checklist.

•Warning = Similar to Failed, but these are for non-critical questions that failed and do not cause the inspection to fail. For example, if the wheel/tire has pieces missing that could cause a bumpy ride. This is something to be aware of, but may not preclude the operator from using the truck.

•Locked out = This occurs when a critical checklist question failed. The supervisor must swipe their badge to unlock the truck.

Checklists Reports

Checklists reports show checklist inspection data and responses, and show how well the data and questions are being interpreted by the operators. The Checklists reports are listed below:

This column chart report indicates the number of checklist inspections that were passed, and failed by operators. It displays the percentage of passes and failures accumulated over the selected date range.

This column chart report indicates the number of checklist inspections that were passed, failed, warnings, timed out and locked out by operators. It also displays the percentage of passed, failed, warnings, timed out and locked out inspections accumulated over the selected date range.

This column chart report counts the number of times each checklist question has had failures and warnings.

This list report counts the number of times each checklist question has had warnings, failures, or has been locked out.

This graph chart report groups selected checklist results by vehicle to help determine which vehicles have a higher occurrence of certain checklist results.

This data grid report provides a detailed look at the forklift truck inspections. It shows when a truck passed, timed out of, etc. the checklist questions.

This list report provides a detailed look at post impact checklists, and information about the impact that occurred prior to the post impact checklist.

This list chart report shows the same data as the Survey-Vehicle Report except in list view form.

This data grid report helps to identify more detailed patterns in trucks and operators failing questions, and summarizes the number of times a truck has "failed" an inspection question.

This data grid report provides a summary of operators and trucks, and the number of times an operator on a specific vehicle type has failed inspection questions.

This list detail report lists the details of each time the question was answered as a failure.

This line chart report displays the percentage of inspection types a vehicle has received over time such as passes, failures, and timeouts.

This list chart report shows the results of the checklists that were completed for the selected vehicle (Unit #).

This list chart report shows detailed information about the checklists that were completed by the selected operator.
